By continuing to register your child you are agreeing to the medical release statement below:
I understand that in the event that my child requires medical or dental treatment while engaged in the activity, reasonable efforts will be made to contact me; however, if I cannot be reached, I hereby consent and give permission to the ministry’s sponsor acting on behalf of Millbrook Baptist Church with respect to the activity, as agent for me, to consent to any treatment advised and supervised by a physician , surgeon, or dentist licensed to practice under the laws of the state. To the best of my knowledge, I have listed above all of my child’s medical allergies, medications being taken, medical problems and/or other pertinent information. I also release and agree to hold harmless Millbrook Baptist Church & VBS leaders from any liability and I assume all risk of injury, damage, or expenses as the result of participation in activities in VBS.
