PERMISSION SLIP
I, being the parent or legal guardian of the child(ren) listed above, give my permission for him/her to attend First Chinese Baptist Church’s AWANA. I authorize my child to receive emergency medical treatment provided by a staff member of AWANA, of the First Chinese Baptist Church of San Antonio, or a medical facility. In case of such emergency medical treatment provided, I agree to be responsible for all actual fees and costs incurred. I agree to release the AWANA staff members and the First Chinese Baptist Church of San Antonio from any legal and/or financial liabilities if my child suffers bodily injuries while on the premises of the Church. I consent and agree that First Chinese Baptist Church of San Antonio, its employees, or agents have the right to take photographs, videotape, or digital recordings of my child(ren) listed above and to use these in any and all media, now or hereafter known, and exclusively for the purpose of First Chinese Baptist Church of San Antonio’s ministry. I do hereby release to First Chinese Baptist Church of San Antonio, its agents, and employees all rights to exhibit this work in print and electronic form publicly or privately. I waive any rights, claims, or interest I may have to control the use of my identity or likeness in whatever media used. I understand that there will be no financial or other remuneration for recording me, either for initial or subsequent transmission or playback.
