INFORMATION:
Kindergarten-8th grade MUST pre-register BEFORE June 5th, so we have enough room, volunteers, and supplies for the children.
A Pre-School class will be available for those who are volunterring & attending the Adult Class. Maximum number of pre-k is 8 children, so register early.
There will be NO 9th - 12th grade (high schoolers) class this year as we are encouraging them to be volunteers for VBS so we can mentor them on ministeriing to others.
An Adult class will be held from 7pm-8:30pm each night.
~VBS Waiver~
By enrolling your child for Vacation Bible School (VBS) at First Baptist Church of Polk City (FBCPC) the parent/guardian gives permission for your child to participate in any and all activities related to the VBS program. Parent/Guardian also agrees to hold FBCPC, the pastor, emplyees, and volunteers NOT responsible from any and all liability, claims, or demands for accidental person injury, sickness, or death.
Medical - In the event the child suffers illness or injury that requires medical attention, the parent/guardian will be contacted as soon as possible by the phone number given upon registration. Parent/guardian authorizes FBCPC and/or adult leaders to exercise reasonable judgement as to appropriate medical care for the child. All costs and expenses incurred will be the responsibility of the parent/guardian.
Photographs/Videos - Parent/guardian agrees to allow photos and/or videos be taken of the child during the course of VBS.
