Ark Encounter/GWL

INFORMATION:

Our trip to the Great Wolf Lodge and the Ark will be Wednesday July 12^th-13^th. We will leave at noon on the 12th from the Gospel Mission, arrive at GWL in Mason, Ohio and spend the rest of our day at the water park! Thursday morning we will get up and travel to the Ark. When we are finished at the Ark we will grab lunch and head back towards Rockport!

Waiver Agreement (Please Agree on the Registration Form)

I, as parent/ guardian of below signed minor, hereby hold harmless and release all volunteers, staff, officers, and The Gospel Mission church from any damages that may result due to accident or injury including transportation to, from and during event. I, the undersigned, also hereby authorize emergency medical treatment, surgery, or dental care to be given to listed minor as considered advisable or necessary in the judgement of an emergency medical professional or attending physician.
