IMPACT Kids! is a fun, interactive, and effective after-school-program for children. When: Tuesdays from 3:30 - 5:00 in the Stratford High School Gym foyer. Who: 1st - 6th Grade Students
At the end of the registration, you will be asked to sign this application electronically. You agree your electronic signature is the legal equivalent of your manual signature on this form. You are agreeing that you have read and understand the statements listed and you are the parent/guardian of the child indicated.
You will be asked to sign the Media Release Statement which is as follows:
During the year, Next Generation Impact/Impact KiDS! volunteers and staff may take photos or videos during the Impact KiDS! Program. These may be used for promoting Imapct KiDS! on a church website or at www.NextGenerationImpact.org.
I hereby give my consent to Next Generation Impact and its designees to photograph and/or video record my child.
I understand that photograph & video recordings become property of Next Generation Impact.
I release Next Generation Impact, its Board of Trustees, agents, employees or other representatives from any liabilities, known or unknown, arising out of the use of these materials.
I waive any and all present or future compensation rights to the use of these materials.
I agree my child's photograph and/or video recording may be used on any internal or external outlet for promotional activities determined by Next Generation Impact & any subsidiaries. This includes all electronic media formats now existing or in the future created.
