Personal Information

Thank you for your interest in volunteering for Summer Spectacular 2025. To be eligible to serve you need to be a regular attender of Calvary Church. In addition to filling out this LifeWay form, all volunteers must complete the following requirements: ADULTS must have a current Kids Life Volunteer Application and current Background check on file. If we need either from you, we will send an email with a link to complete one. TEENS: A current Kids Life Student Volunteer Application and a current Student Life Health/Liability form needs to be on file. We will contact you if any forms are needed. All volunteers are subject to the final approval of the Kids Life Director.
