This is a drop off VBS - NO PARENTS OR GUARDIANS ARE ALLOWED TO REMAIN ON THE PROPERTY DURING VBS
Photo Release: By allowing your child to participate in Gospel community Church's VBS, I the undersigned, by checking yes, am giving permission for my child/ren's photos to be used on Gospel's webpage and/or Facebook/Internet.
Liability Release: In consideration of Gospel Community Church allowing the child to participate in it's VBS program, I, the undersigned, by checking Yes, do hereby release, forever discharge and agree to hold harmless Gospel Community Church, its pastors, directors, employees, volunteers and teachers (collectively herein the "Church") from any and all liability, claims, or demands for accidental personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred but the undersigned and the child while involved in its VBS program. Furthermore, I, on behalf of my minor child, hereby assume all risk of accidental personal injury, sickness, death, damage, and expense as a result of participation in recreation and other activities involved therein. The undersigned further hereby agrees to hold harmless and indemnify said Church for any liability sustained by said Church as the result of the negligent, willful, or intentional acts of said child, including expenses incurred attendant thereto.
